These are important dates for you to know regarding this Settlement:

• December 28, 2012 Preliminary Approval Date


• May 13, 2013: Deadline to Object to the Settlement (must be received by this date)


• May 13, 2013: Deadline to File Notice of intent to Appear at Fairness Hearing (must be filed by this date)


• May 13, 2013: Deadline to Request Exclusion from the Settlement (must be postmarked by this date)


• June 14, 2013 9:00 a.m.: Fairness Hearing


• July 29, 2013: Deadline to Submit a Claim
