Important Dates

These are important dates for you to know regarding this Settlement. (Please note that these dates have all passed and, while the Court has granted final approval to the settlement, it is still subject to appeal.):

• December 28, 2012 Preliminary Approval Date

• May 13, 2013: Deadline to Object to the Settlement (must be received by this date)

• May 13, 2013: Deadline to File Notice of intent to Appear at Fairness Hearing (must be filed by this date)

• May 13, 2013: Deadline to Request Exclusion from the Settlement (must be postmarked by this date)

• June 14, 2013 9:00 a.m.: Fairness Hearing

• July 24, 2013: Court entered Final Approval Orders

• July 29, 2013: Deadline to Submit a Claim

• March 14, 2014: Commenced mailing Distribution Payments to eligible class members that filed timely claims

• July 7, 2014: Commenced mailing Distribution Payments to eligible class members that filed claims and timely resolved deficiencies in their claims

• September 29, 2014: Commenced mailing Distribution Payments to eligible Class Members

• March 9, 2015: All checks that had an issue date of September 29, 2014 are now stale

• April 20, 2015: Commenced mailing the reissue of all uncashed checks that had a March 9, 2015 void date

• August 17, 2015: All checks that have an issue date of April 20, 2015 will become stale
